Transplant is a brand new medical drama on NBC hailing from our friends in Canada. It tells the story of a Syrian refugee who finds himself in Canada working in a restaurant and then getting thrust back into life at the hospital after his career as a Syrian doctor resurfaces. It’s slickly produced and a dramatic premise, but how medically accurate is it? From intubations to the Heimlich maneuver, to the bizarre operations of this level 1 trauma center, I have some things to say!

  • My bf was in a car crash and got pierced/stabbed by a piece of car. He knew it wasn't hitting anything vital so he just sat there. When people tried to help him pull it out he told them not to and to just wait for the ambulance. So glad he didn't try to pull it out!

  • Wait, so the precordial thump is used in case of v fib? So not when the heart stops, but it's actually a real thing? An emergency substitute for a defibrillator if you're absolutely sure it's necessary and you don't have one around. And even then, it's supposedly only effective if you catch the fibrillation within seconds of its onset. Needless to say you can end up doing more harm than good, including killing the patient.

  • War zone medicine is different than hospital medicine... They don't always have the luxury of tests and confirmations, they have to rely on experience and intuition in any given moment. Bashir is coming from war medicine.. there's going to be things he does that don't make sense to hospital doctors. That's the main dynamic between him and the others The precordial thump, while not used often in nice clean hospital conditions, and may be considered not very effective, it most certainly is used in war/field medicine just because it's all they have. the drilling into the man's head to relieve pressure; seeing the effects of something enough - he can make a call/highly educated guess and have a high chance of being right... It needs to be kept in mind, his main experience is from war zone Syria with lack of resources, everything is complete chaos, high risk, on the fly, from the gut.. they don't always have Labs, defibrillators, CTs, MRIs or x-rays. Everything he recognizes and 'diagnoses' without tests is stuff he's seen so much of, he knows it when he sees it... that needs to be acknowledge to give the character *some* due respect, even if "just a tv show" "We don't just punch people in the chest" you would if you're in the middle of nowhere, countless people needing help and there's you and maybe 5 other medics, with bombs and/or gun fire going off around you, or you're at an aid station near frontlines and you have to get a pulse before sending them on to a MASH unit.... even if found to be less effective than all the things you listed, if you had none of those things's more effective than nothing, or when compressions are taking too long. you're literally just desperate to keep this person alive, you'll do anything you can possibly think of. One of the things with the jumping to whatever, without tests/confirmations ect, is that it's a 45 min drama... to portray anything ultra-realistically would be too much for what it's meant to be.. I would hope most viewers understand this.. all in all though, I'm really enjoying these Doctor reacts to _______ videos.. I'm not sure how many I've binged so far, they're fun to watch :D This one, I just want it to be clear where Bashir is coming from.. War/Conflict/Field is just .... different ...

  • If anyone came to the comments out of curiosity as to why the main character punched the lady in the chest, I have an answer. There was a manuver that used to be taught called a cardiac thump, where a medical provider hits a patient in the chest if they witness them go into cardiac arrest. It's super outdated, and I'm pretty sure they stopped teaching it in the 80s or 90s when they realized that high quality chest compressions and early defibrillation are way more effective. The doctor in the show is definitely too young to have learned it in med school tho

  • The only thing I can think of as to why the guy punched the woman in the chest was because he knew there were multiple people to save around to save, who likely had a better chance, he was triaging, and rather than straight up leaving her, and going onto the next patient, just tried it, if it failed, she dies, but if he triaged and left her, then she would also die. Seemed like a no lose strategy. Triage in the field in an emergency is the opposite to triage in hospital. You get those most likely to survive stable first to save the largest number of lives.
